Burlington Extra Duty Detail Application

Rates:

Tier One rates (customers requesting service 6 or more times per year) are:
Regular: $34.80 / hour
Alcohol Rate: $40.60 / hour
Premium/Holiday*: $46.40 / hour (Customer can choose to pay this rate to attract Officers; 7 days or less notice)
Premium/Holiday* Alcohol: $52.20 / hour (Customer can choose to pay this rate to attract Officers; 7 days or less notice)
Supervisor: add $5.80 to the rate (5th Officer must be a Supervisor)

Tier Two rates (customers requesting service 5 or less times per year):
Regular: $40.60 / hour
Alcohol Rate: $46.40 / hour
Premium/Holiday*: $46.40 / hour (Customer can choose to pay this rate to attract Officers; 7 days or less notice)
Premium/Holiday* Alcohol: $52.20 / hour (Customer can choose to pay this rate to attract Officers; 7 days or less notice)
Supervisor: add $5.80 to the rate (5th Officer must be a Supervisor)
Cruisers: $20 per day

A minimum of 2 Officers are required when alcohol is being served

Customers must cancel within 24 hours or there will be a 3 hour minimum payment for each Officer scheduled.

*Holidays – New Year’s Day, Martin Luther King Jr.’s Day, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Halloween, Veteran’s Day, Thanksgiving Day, Christmas Eve, Christmas Day.

I have read and understand the Terms of Agreement. I agree to abide by Burlington rules, regulations and ordinances should my application be approved. Additionally, I will fulfill the requirements placed upon this application as expressed in the Terms of Agreement.
